(I assume you're using cPanel, with phpmyadmin and mysql databases

- Go to Mysql Databases in your cPanel

* Follow the instructions

* Add the user to the database

- Go to phpmyadmin

- Click your database name, that you create in mysql database

- Scroll down, you should see the following form

- Fill it out

- Press "Go"

- & fill in the column names
