[REQUEST]Need a login page coded

I have a login/register I created in GIMP. Can be saved as a PSD if needed.

I need it coded, as I have no idea how to do so.

It would need to be a fixed width matching it's current dimensions, with a repeating background, and the banner sitting directly on top.

If you also happen to know how to work with Lightbox, I would also like two screen shots included.
